To obtain information on licensing rate service organizations, please call (804) 371-9331 in the Forms and Rates Section of the Property and Casualty Division of the Bureau of Insurance.
Life and Health, Property and Casualty, Fraternal Benefit Societies, or Title insurers seeking licensure in Virginia should use the Uniform Certificate of Authority Application. Newly formed Virginia Life and Health, Property and Casualty, Fraternal Benefit Societies, or Title insurers seeking a license in Virginia should use the UCAA Primary Application. (UCAA Primary applicants should not file an application in duplicate. Only one copy is required.) The UCAA Expansion Application should be submitted by all other Life and Health, Property and Casualty, Fraternal Benefit Societies, or Title insurers already licensed in another state.

Regarding NAIC Biographical Affidavit Forms
Most of the applications require the officers and directors of the applicant to complete the NAIC biographical affidavit. To obtain the most current version of the NAIC biographical affidavit form, visit the NAIC's Uniform Certificate of Authority Application (UCAA) website.

When the Bureau of Insurance receives an application, the Supervisor of the section or a designated person will briefly review the application to ensure all of the required documents have been submitted. If the application is deemed complete, the application will be accepted.
If additional information is required during the application review process, a letter will be written to the applicant. Once all information has been filed, and the appropriate levels of review are complete, a decision is made to approve or disapprove the application, and the applicant will be notified in writing.
