The Division of Communications has made its Telephone Service Inquiry/Complaint Form available via e-mail. Please complete the form in its entirety. The Division of Communications may require additional information for processing. If additional information is required, the Division of Communications will notify you and provide you with an identification number to be be referenced on all subsequent correspondence.

Supplemental information (copies of documents, letters, or other relevant materials), if required, must include the identification number, and should be mailed to the Division of Communications, P.O. Box 1197, Richmond, Virginia 23218 or faxed to 804-371-9069.
