The authorization form is available for download in two formats.

Both the Microsoft Word document and the Adobe PDF file contain text fields. Please use your "tab" key to navigate through the documents.
The PDF version requires
that you have special software to view it. If
you do not have the Acrobat Reader, you can download if for free from
the Adobe website.
