Regarding NAIC
Biographical Affidavit forms

To obtain information on licensing and
approving other company types or to
obtain an application packet, please call 804- 371-9546.
When the Bureau of Insurance receives an application, the Supervisor
of the section or a designated person will briefly review the application
to ensure all of the required documents have been submitted. If
the application is deemed complete, the application will be accepted.
If additional information is required
during the application review process, a letter will be written
to the applicant. Once all information has been filed, and the appropriate
levels of review are complete, a decision is made to approve or
disapprove the application, and the applicant will be notified in
writing.
