The Clerk's Office has produced a large number of forms to help prepare documents that will meet the requirements for filing under Virginia law. Many of the forms are available in pdf-fillable format. Note that the formation of new Virginia stock corporations and LLCs can be accomplished online in real-time at SCC eFile.

Expedited Service Request Form
The processing of most of the forms found above may be expedited for an additional
fee when submitted with an Expedited Service Request Form. For more information,
see Expedited Services.

Forms Request page to request one or more of the following Commission-prescribed forms:
- Notice of Assessment of Annual Registration Fee
- Corporate Annual Report
- Statement of Change of Registered Office/Registered Agent
- Statement of Change of Principal Office Address of a LLC
SCC eFile may be used (i) to pay an annual registration fee for a corporation or limited liability company, (ii) to file a corporate annual report that does not change director or officer information, and (iii) to make registered agent, registered office, and LLC principal office address changes online.
Notice Regarding Personal Identifiable Information
Personal identifiable information, such as a social security number, should NOT be included in a business entity document submitted to the Office of the Clerk for filing with the Commission. For additional information, see the Notice on the Clerk's Home Page.
