Forms and Fees

The Clerk’s Office has produced a large number of forms to help you prepare documents that will meet the requirements for filing under Virginia law. Most forms can be downloaded as a Word document to your computer, where you can make the adjustments you need or desire.

Expedited Service Request Form

The processing of most of the forms found above may be expedited for an additional fee when submitted with an Expedited Service Request Form. For more information, see Expedited Services.

Commission Prescribed Forms

The Code of Virginia mandates the use of a Commission-prescribed form for corporate annual reports, changing a registered agent or registered office, and assessing annual registration fees. To request the generation and issuance of one of these forms from the Office of the Clerk, visit the Electronic Forms Request page.

Important Notice Regarding Personal or Confidential Information

Information such as social security number, date of birth, maiden name, or financial institution account number is NOT required to be included in business entity or Uniform Commercial Code documents filed with the Office of the Clerk of the Commission. Any information provided on these documents is subject to public viewing.
